2 Questions for SimChart Clinical Assignment 1. SBAR- Jana Green S- Hello, my name is Emily Serna I am the nurse for Jana Green in room 551. Mrs. Green’s primary complaint is unrelieved pain primarily felt in her back. Ms. Green reports pain level as an 8 out of 10 that is constant, sharp and is aggravated by movement. B- Jana Green is a 76-year-old woman who was directly admitted from her primary healthcare providers office. She has a history of lung cancer with bone metastasis. Mrs. Green has been noted of being dehydrated upon admission. A- I have been continuously monitoring Mrs. Green’s vital signs and documenting a thorough pain assessment. Mrs. Green has also received administration of hydromorphone via PCA infusion however she has reported that her pain has not been relieved with the current medication dose. R- Mrs. Green would benefit from an increase in the dose of the hydromorphone infusion. I will continue to monitor vitals and her pain level. I will assess how the medication is affecting her pain as well. I will also advise Mrs. Green on nonpharmacological interventions to treat pain if desired and keep patient as comfortable as possible. 2.
