COUNSELLING
The consultation process is fundamental to the provision of pharmaceutical care and is key to the successful identification of a patient’s pharmaceutical care problems. It consists of: An introduction phase, whereby the practitioner engages with the patient A data gathering phase, whereby relevant information is obtained from the patient followed by an identification of a patient’s pharmaceutical care needs. A discussion on how to prevent or resolve the problems identified and to negotiate shared management strategies A closing phase whereby the practitioner: o Determines whether the patient needs additional information o Discusses contingency plans in case something goes wrong o Negotiates follow-up plans
