Introduction
Environment o Plinth height / brakes on bed
Infection control o Wash hands
Explanation o Breathing techniques to clear secretions
CONTRAINDICATIONS o No contraindications but monitor patient carefully throughout
Consent
Position of patient o Long sitting – supported with pillows o Abdomen is unrestricted by tight clothing
Treatment o Normal breathing control – relaxation of upper chest and shoulder girdle o Thoracic expansions – maximal breath in and hold for 2 seconds, breathe out gently. No more than 4 to avoid hyperventilation. Place hands on ribs to encourage expansion o Back to normal breathing – explain huffing o Long huffs – normal to mid-volume breath in, long huff out. Demonstrate ‘steaming up a window’. Repeat 2 – 3 times – pot available for secretions. o Back to normal breathing o Short huffs – maximal inspiration followed by a short sharp huff.
