Description of Skill This is a quick physical exam of the newborn to rule out any abnormalities.
Indications From Birth the newborn assessment needs to be completed at 1 and 5 minutes of life. The apgar score is based on a total score of 1 to 10. The higher the score the better the baby is doing after birth!
Outcomes/Evaluation The newborn demonstrates adaptation to being outside of the uterus (extra-uterine life).
Potential Complications Respiratory distress if respirations haven't been established.

CONSIDERATIONS
Nursing Interventions (pre, intra, post) Make sure that the resuscitation equipment is available prior to the mother delivering. Assess the newborn's HR, RR, Muscle tone, reflex irritability and skin color. Ensuring that the respirations are established. Dry the infant and assess temperature.
Client Education Inform the parents of what the infant's score is, interpret the meaning of the score. Encourage kangaroo method (Skin to skin contact)
