vSim Health Assessment Case 1: Josephine Morrow Guided Reflection Questions Opening Questions How did you feel throughout the simulation experience?

This sSim was a little different than what I’ve been used to so far with the other vSims. This one was a little more interactive with the skills to perform than the previous vSims so far. I was getting a little frustrated because I was performing task that I have normally done but was timing out. Then I realized that certain task didn’t need to be done at the moment. I needed to worry about what was needed to be completed not what I’m used to doing. What do you think went well?

The skin assessment went well because I knew from the recommendation that a full detail assessment is what was being asked for the nurse to perform. Scenario Analysis Questions* PCC What priority problem(s) did you identify for Josephine Morrow? 

Priority problem that I identified was her having any more pressure sores and the poor circulation. Because of her lack of mobility, we want to keep her active as much as possible with some ROM and ambulating when we can. We also want to make sure we are keeping an eye out on her skin integrity and documenting any changes that can cause her any harm. What potential problems could arise due to the venous stasis ulcer and comorbidities?

PCC 

Potential problems that could arise due to venous stasis ulcer and comorbidities, could be her developing a DVT she already has a history of having them. She. Could also develop an infection if not taken care of properly. So, we want to elevate her legs, ambulate her to get the blood circulating in her legs and compression stocking on the unaffected leg until wound is healed or with a dressing in place. Discuss your initial assessment findings related to the integumentary system for Josephine Morrow.

hair on her scalp was thinning, but scalp was clean and dry. The skin at the thorax was smooth, warm, dry and mobile with no tugor. The skin at the upper arm had no lesions or scarring; normal color, odor and skin integrity. Which labs are related to the integumentary health of Josephine Morrow?

Albumin, prealbumin and protein labs were related to Josephine Morrow. Keeping an I on these levels helped us to how the patient was healing. What is the priority intervention to prevent further compromise of the integumentary system for Josephine Morrow?

The nurse assigned to her, the aids, her doctor, physical therapist, a social worker and a dietitian. Describe the safety measures that should be incorporated in Josephine Morrow’s care.

Opportunities for improvement would be to making sure I’m completing the right task that I’m assigned for the amount of time that is given.
