CLINICAL GRADING SYSTEM: Student clinical experiences are graded weekly on a numerical scale from “0-5". . A point average of “2" is required to pass NUR 112, NUR 113 . A point average of “3" is required to pass NUR 114, NUR 115, NUR 211 C. A point average of “4" is required to pass NUR 221 The following behavioral criteria are used to determine the numerical grade for individual elements of the evaluation: . The instructor will assign a score of “5" for any behavior where the student is performing at a level that far exceeds the expected level. The student must evidence exceptional behavior to receive a score of “5". II

V. The instructor will assign a score of “1" for any behavior where the student is performing that behavior at an “unsafe” level using an inadequate knowledge base. For example: exhibits a beginning understanding of expectations as outlined above but student shows evidence of need to improve in specified areas before reaching passing criteria. ********************************* VI. The instructor will assign a score of “0" for any behavior where the student’s performance or knowledge base poses a “ serious threat” to patients, the profession, the facility, or the program, for example: A. Data lacks depth, appropriateness or accuracy B. Behavior is unaccountable or lacks consistency which may place the patient in jeopardy C. Student is not gaining awareness of own strengths and weaknesses in specified areas D. Has difficulty adapting to change or is easily distracted from commitment to competent clinical performance. E. Documentation is superficial F. Lacks motivation to seek learning experiences and continually needs faculty to guide experiences

********************************* NOTE: If at any time a student performs in an “unsafe” manner which jeopardizes client safety, the instructor has the authority to “fail” the student at that time. The student must receive immediate feedback (verbal/written) regarding the specific behavior in question. This evaluation tool, along with a written description of the incident/s will be signed by the student and faculty member and placed in the student’s file after proper notification is given to the full time nursing faculty responsible for the course. Each clinical week, the instructor evaluates the student using the numbers “0-5" as defined on pages 1-3. A detailed explanation will be given the student for every “0" or “1" in order for learning to occur. Each week the total point average will be computed and documented and the student will receive the evaluation. When total points are below the expected level, the student will be counseled by the instructor regarding the area/s needing improvement. At mid-term/semester’s end, points are averaged and documented and the student informed of clinical status, ie. (PASS/FAIL)

BEVILL STATE COMMUNITY COLLEGE NURSING PROGRAMS CLINICAL EVALUATION POLICY The clinical evaluation tool will determine the student’s clinical competency skills required for progression to next nursing course in the curriculum. This will be a Pass/Fail evaluation according to

the adopted evaluation tool for each course which is given to the student/s on the first day of class. 1. The clinical component for this course must be passed for progression to the next nursing course. See “Bevill State Community College Nursing Program Policies” for additional information. 2. An assigned numerical point average is required to pass the clinical component of each course. If a passing score is not achieved on the “Student Clinical Evaluation” tool, a grade of “F” will be received for the entire course. (See Clinical Evaluation tool for complete information and criteria measured). 3. If a client is harmed directly by the student's action or inaction, the student will be dismissed from the program at that time. 4. All clinical grade decisions relating to student's written work, knowledge regarding client condition, accountability and responsibility, etc. are made at the discretion of the clinical instructor and the nursing faculty. 5. If a student presents for clinical unprepared to give the patient care, the instructor will require the student to leave the clinical area and this will count as an unsatisfactory clinical day and will result in a grade of “0" for the day. 6. Failure to notify the clinical area and/or the instructor 30 minutes prior to the clinical time for an absence or tardiness will also result in a “0" grade for the day being given. THERE WILL BE NO CLINICAL MAKEUP DAYS PROVIDED FOR UNEXCUSED OR EXCUSED ABSENCES. 

A ZERO WILL BE GIVEN FOR ANY UNEXCUSED CLINICAL ABSENCE AND THIS WILL BE AVERAGED IN WITH THE OVERALL CLINICAL PERFORMANCE.

7. The student/s and the clinical instructor are responsible for notifying the course coordinator of any clinical absence during the semester. 8. An unsatisfactory clinical performance (“0" grade) will be given for any action (or lack of action) that creates an unsafe environment for, or endangers, the patient.

1 Clinical Objective 1: Utilize the nursing process and critical thinking to make clinical decisions in collaboration with assigned patient with common health problems to meet basic illness and wellness needs. (Clinical Judgement; Spirit of Inquiry) Perform focused assessments and interpret relevant data. Provide care that is adequate, applicable, and prioritized to include patient education and discharge planning. Perform psychomotor skills according to predetermined standards. Implement interventions that are based on available evidence and best practices. Promptly meet the needs to 2 patients in an inpatient adult health setting. Clinical Objective 2: Communicate effectively with the patient and members of the health care team to ensure adherence to the patient’s collaborative plan of care. (Teamwork & Collaboration; Nursing Judgement, Safety) Seek information from members of the interdisciplinary team to collect data and ensure adherence to the patient’s collaborative plan of care. Coordinate care for patients to create a culture of safety. Provide for safety, comfort, and well-being of patient. Coordinate continuing care with the patient, family, case manager/discharge planner, and other health care team members as needed. Seek internal and community resources to assist in health teaching to meet the patient’s and/or family’s learning needs. Use electronic medical record, medical databases to skillfully access patient data and current research; communicate essential information as needed.

Clinical Objective 3: Document care appropriately and accurately using the electronic patient record and/or other medical record format (Communication) Document care consistent with protocols of the facility and best practices. Ensure that documentation is clear, concise, complete, relevant and accurate. Promptly respond to and document/report changes in a patient’s condition.
