Description

Postpartum Assessment B-Breasts-Inspect and palpate breasts and nipples. Discuss breastfeeding how it is going. Educate client on normal breast changes and answer breastfeeding questions as needed.

U-Uterus-Perform fundal check and assess for location and firmness of uterus. Educate client on normal involution of uterus over next several weeks. At this point, if the patient was a C-section you should also assess the incision site. B-Bowels-Ask patient if she has had a bowel movement or passed gas. Educate on importance of keeping stool soft and issues surrounding constipation.

B-Bladder-Assess for full bladder when palpating uterus. Ask patient when she last urinated. Ask if urination was painful.

L-Lochia-Assess amount and color of lochia noted on pad. Educate patient on average lochia flow. E-Episiotomy- Have patient turn on side, assess episiotomy or tear for redness, swelling and erythema. Make sure the area is well approximated. Educate patient on care of episiotomy and tear. Encourage ice, ask about pain level.

H-Homan’s Sign-Since we do not do Homan’s sign, we need to assess bilateral lower extremities for pain, swelling warmth or redness.

E-Emotional Status-Assess for maternal infant bonding. Assess and educate patient and family on signs and symptoms of postpartum depression. Assess social support of patient.
