Capstone Project: Evaluation and Dissemination Plan Evaluation of the change plan The use of change theories remains instrumental in altering systems within health care organization systems and the community. In addressing the issue of antimicrobial resistance (AMR) within the hospital setting and in the community, change theories such as innovation diffusion theory and force field and the unfreezing-change-refreezing model will be used. The review of the logic model, change proposal, and the initiation plan will be a necessary step in designing a dissemination plan. For instance, in achieving the project outcomes of reducing disease infections, increasing disease control strategies, and overall prevention of AMR, the project will integrate Roger’s five –steps of innovation diffusion theory and Lewin’s three-steps of change model Wojciechowski, Murphy, Pearsall & French, 2016. As nurses are important to change agents, their skills and behaviors largely influence organizational and community actions throughout the process of change. Therefore, nurses will play a critical role in the change plan both in the evaluation stage and the project dissemination step to ensure the successful implementation of the project. According to Kurt Lewin’s theory of unfreezing-change-refreezing theory, the behavior is a product of balanced forces that work in opposing directions (Wojciechowski, Murphy, Pearsall & French, 2016). Lewin’s change model depends on both the resistance and driving forces from employees and other nurses oppose or support the proposed change. Roger’s change theory is a modification of the Lewin’s theory to comprise of five steps: knowledge, persuasion, decision, implementation, and confirmation (Mohammadi, Poursaberi & Salahshoor, 2018). The refreezing stage of Lewin’s theory and the confirmation stage of Roger’s model involves finalizing and accepting change as a new habit. At this stage, the community and health care

organizations will adopt recommended measures to prevent AMR. Deriving from the logic model, implementation of the change plan will require human resources such as nursing staff for training and monitoring, surveillance and educational resources, and essential medicine and water supply within hospitals and the community. Dissemination plan of the project outcomes While evaluation of the plan aims to assess the success of the change plan throughout the implementation stages, the dissemination stage is the final step of the project and will need the participation of health care workers, patients, government, and other non-government institutions, and the community. During the dissemination plan, it is important to consider the short, medium, and long-term project outcomes or goals. As short-term and medium-term objectives, stakeholders will gain the knowledge and skills in addressing the issue of AMR and reduce cases of infections. Long-term objectives will be achieved with the complete eradication of the issue of AMR and it might take a longer duration such as 10-20 years. This is because some strategies such as vaccine development to address the issue of AMR might take longer to develop and make the treatments available for populations (Lurie, Saville, Hatchett & Halton, 2020). However, short-term objectives such as ensuring that stakeholders have the necessary knowledge and skills in dealing with AMR will take shorter periods to implement. Depending on the resistance from other staff to the change model, the stage of change dissemination might be delayed. However, integrating the two theories of change model above will help in identifying the stages that are likely to take longer and have strategies in place to culminate the effects of resistance. For example, the refreezing stage of Lewin’s theory demands that stakeholders adopt a completely new habit, which will take time, but is a necessary step in

CAPSTONE PROJECT ensuring change implementation (Hussain et al., 2018). However, this depends on the effectiveness of the persuasion stage in Rogers’ change model.
