Outcomes

Patient reports any altered sensation or pain at site of tissue impairment. Patient’s wound decreases in size and increased granulation





Interventions



Monitor site of impaired tissue integrity at least once daily for color changes, warmth, pain or infection. Provide tissue care as needed.



Impaired verbal communication related to lack of speech





Physiological health care like playing with patient or read the book. Improved communicati on and activity through physical exercise, improving range of motion





Patient will learn new methods of communic ation through playing games and reading books Patient will have improved social skills and communic ation through interaction s during game play.

Evaluation

Skin texture and quality improved, skin shiny, moist and not overly dry. No areas of skin breakdown on evaluation No areas of irritation, eczema on skin evaluation

Patient able to express himself in other ways besides verbal communication . Patient able to interact with others, express desire to play, desire to stop, necessity to use restroom, convey happy sad while interacting
