Infant History Evaluation Goals 1. Client will demonstrate necessary knowledge needed to provide adequate care of her infant before discharge. 2. Baby will have a fully healed circumcision within 14 days. 3. Patient will follow directions and/or actively participate in education demonstrations.

1. Goal met, client was an expert before discharge. 2. Goal met, no signs of infection. 3. Goal met, family actively participated in demonstrations and felt confident after.

Mother History

Reproduction: Nursing Diagnosis 1. Deficient knowledge R/T unfamiliarity with resources AEB request for addition information. 2. Acute pain R/T trauma of tender tissues AEB crying, irritability, & changes in sleep patterns. 3. Anxiety R/T situational change AEB jitteriness.

Nursing Interventions 1 1. Assess the client’s level of knowledge, ability to learn. Provide time for questions and clarifications. 2. Talk and listen to the client in a calm demeanor. 3. Explain newborns care to mom in terms that she will understand. 4. Demonstrate care on infant and watch mother repeat the demonstration.

APGAR 6 @ 1 min & APGAR 9 @ 5 min. Vitamin K injection and erythromycin ointment. Circumcision performed. B+ blood type.

Postpartum

Nursing Interventions 2 1. Avoid alcohol and soap on penis while healing. 2. Apply petroleum and gauze to penis and loosely wrap diaper. 3. Provide pacifier (dipped in sugar if needed) and lightly rub infants back and speak calmy to them. 4. Administer acetaminophen as directed.

R.B. 29yr old G1P1, vaginal delivery to a male infant 8lb 5oz. She is O- blood, Hep B-, Rubella immune, RPR non-reactive. 2/10 pain, very interested in infant care info.

Nursing Interventions 3 1. Maintain a calm, deliberate manner. Offer clear and concise instructions. Provide explanations. 2. Encourage partner or SO to remain with the client, provide support and assistance as needed. 3. Encourage appropriate coping or relaxation techniques. 4. Provide a quiet environment and privacy within parameters of the situation. Position client for optimal comfort....
