The services offered must be the least intrusive possible into the life of the parent and child, must focus on clearly defined objectives, and must provide the most efficient path to quick reunification or permanent placement given the circumstances of the case and the child’s need for safe and proper care. In the case of Christopher Bowen not being about to be reunified with his biological parents, the Department will seek permanent determination by the courts, in accordance with Florida Statute 39.621 and continue care, in accordance with Florida Statute 39.6251. II.

DANGER STATEMENT

A description of the identified problem being addressed, including the parent’s behavior or acts resulting in risk to the child and the reason for the intervention by the department. FLA. STAT. § 39.6011(2)(a) (2017). Child Protective Services (CPS) was notified about Christopher Bowen (Christopher) in regards to concerns about Kaylene Bowen-Wright (Bowen-Wright), mother, intentionally causing medical child abuse. Due to efforts of the CPS petition for removal in an affidavit, Christopher, 8-years old is currently in foster care. In compliance with Florida Statute 39.6011 (1)(a), Case Manager developed a case plan after a face-to-face conference with Bowen-Wright, Christopher, Dr. Roper-Dakil, doctor at Children’s Medical Center Dallas, and Ryan Crawford. This report is confidential except for those involved in the case in occurrence with Florida Statute 39.6011(1)(b). The following information was obtained during the conference: Christopher received 13 major surgeries and had 323 doctor visits, which included, but are not limited to radiological exams, receiving feeding tubes, bond to a wheelchair, and suffering lifethreatening blood infections. Bowen-Wright showed signs of child abuse when Christopher was only 11 days old by pouring out Christopher’s milk and lying to hospital staff. Children’s Medical Center Dallas reported concerns that Bowen-Wright was purposely making Christopher sick with the most recent incident being inducing symptoms that could lead to a fatal seizure.

VI. CHILD PLACEMENT INFORMATION If concurrent planning is being used, a description of the permanency goal of reunification with the parent or legal custodian in addition to a description of one of the remaining permanency goals described in Florida Statute 39.01. FLA. STAT. § 39.6011(2)(c) (2017). If a child is removed from a parent and is placed with a parent from whom the child was not removed, the court may leave the child in the placement with the parent from whom the child was not removed with maintaining and strengthening the placement as a permanency. FLA. STAT. § 39.6011(2)(c)(2) (2017). Christopher Bowen was removed from Kaylene Bowen-Wright on November 16, 2017. He was initially placed in foster care and will shortly be reunited with his biological father, Ryan Crawford. In accordance with, Florida Statute 39.6012(3), Christopher Bowen will be living in a single-home house at 201 Beggs Avenue Orlando, FL 32801 with father, Ryan Crawford. The Department has developed the case plan to allow a smooth transition. Should the placement of Christopher with Crawford not present a healthy living arrangement or should the case plan not be followed, the Department will act in accordance with the following Florida Statutes: 39.6011(4)(a); 39.6035; 39.604. VII. CASE MANAGER’S RESPONSIBILITIES The responsibility of the case manager to forward a relative’s request to receive notification of all proceedings and hearings submitted pursuant to Florida Statute 39.301(14)(b) to the attorney for the department[.] FLA. Notwithstanding the participation from the parent(s), in the case plan development, the Case Manager will utilize Florida Statute 39.602. (1) The Case Manager will have contact with the parents at least one time per month if the parents’ locations are known. (2) The Case Manager will have contact with the child at least one time per month and more frequently if necessary. (3) The Case Manager will attend all scheduled court hearings and report progress to the court. (4) The Case Manager will provide the parents with referrals and ensure engagement with services listed in this case plan, including,but not limited to, assisting the parents with any difficulties accessing services. (5) The Case Manager will work with Children’s Legal Services for the Department of Children and Family Services to ensure all required paperwork is submitted in compliance with State and Federal Guidelines. (6) The Case Manager will refer the child to any necessary services and endeavor to see that services are engaged. (7) The Case Manager will ensure that the child living in a safe, clean and stable residence, free of hazardous conditions. (8) The Case Manager will be present during parents visitation hours. (9) The Case Manager will monitor the Tasks/Outcomes to ensure the completion. (10) The Case Manager will ensure the child’s medical safety by confirming the medical plan provided by approved doctors. (11) The Case Manager will comply with Florida Statute 39.602, notwithstanding the participation from the parent(s), in the case plan development. VIII. NOTICE OF CHANGE OF RESIDENCE AND CONTACT INFORMATION The case plan must include all the available information that is relevant to the child’s care including, at a minimum: FLA. (a) A description of the identified needs of the child while in care (b) A description of the plan for ensuring that the child receives safe and proper care and that services are provided to the child in order to address the child’s needs.
