Case study analyzing a school aged client ...

Description

Case Study 1: Jessica Wilson Point of emphasis for case study: Mainstreaming Age: 5 years, 10 months Gender: Female Background information and previous aural rehabilitation: Jessica was identified with a moderate-to-severe bilateral sensorineural hearing loss at age 15 months. Her mother had suspected that she was not hearing very well when Jessica was about 9 months of age, but her pediatrician had assured her that her daughter could hear. Upon diagnosis, Jessica received binaural BTE hearing aids. Genetic counseling indicated that Jessica might have a syndrome characterized by progressive hearing loss, and Jessica’s parents were advised about the prognosis of eventual profound deafness. Mrs. Wilson began to learn total communication, although she never felt very comfortable with her knowledge of sign language. Jessica received aural rehabilitation from an itinerant speech and hearing clinician in her home until she turned 3 years old. By the age of 3 years, Jessica had measurable hearing only for the frequencies 250 through 1,000 Hz. Total communication was her preferred mode of communication. Shortly after her third birthday, she enrolled in preschool at the state school for the deaf. Six months later, Jessica received Advanced Bionics cochlear implants bilaterally. Bilateral cochlear implants were recommended because the cochlear implant team wanted Jessica to reap any possible benefit from having binaural hearing, such as improved listening performance in the presence of background noise. Shortly after she received her cochlear implants, Jessica’s mother decided to transfer her to a private oral school so that her daughter would receive concentrated spoken language instruction. The mother gradually discontinued using her rudimentary sign vocabulary at home, and began to communicate with Jessica solely through spoken language. The family relocated to the Midwest so that Jessica could attend the new school. Now Jessica is approaching the age of 6 years. Mrs. Wilson is interested in having her attend a mainstream elementary school for first grade in the family’s local neighborhood. Jessica’s kindergarten teacher in the private oral school also believes that Jessica is ready for this transition.

Family situation: Jessica lives with both parents. Her mother works part-time as an accountant and her father is a carpenter. She has an older brother and one younger sister, who has also been diagnosed as having a genetic progressive hearing loss.

Work sheet assignment 1. What services or other types of supports might be suggested that would be important to include in Jessica’s Individualized Education Plan (IEP)?

2.

What possible difficulties might Jessica experience in the next couple of years that might require special attention in the educational setting?

3.

How might an audiologist or speech-language pathologist facilitate Jessica’s transition from an oral school to a mainstream setting?...
