Primary Nursing In the space below provide an explanation distinguishing the different models of care within each category. Traditional Total Patient Care was the original model of nursing care where the RN was responsible for all areas of care of one or more patients. Functional Nursing, the needs of a group of patients are separated into tasks that are assigned to RN’s, LPN’s and UAP’s so each caregiver skills are used to the best ability. Team Nursing is a team of nurses that provide total patient care to a group of patients. Primary Nursing was to help place the RN back at the bedside. Integrated Practice Partnership is where an RN and a UAP or LPN agree to be practice partners Case Management become a model for identifying, coordinating and monitoring services needed to achieve desired patient care outcomes within a certain time period Critical Pathways refers to the expected outcomes and care strategies developed by the collaborative practice team Evolving Patient Centered Care is nursing care delivery where the role of the nurse is to coordinate a team of multifunctional based caregivers. Synergy Model is care based on the needs of patients Patient Centered Medical Home is a model for delivering primary care to make sure that

appropriate care is provided to the population of clients. 3. Review roles and responsibilities of a nurse case manger Specific patient diagnoses that represent high-volume, high-cost and high-risk cases are selected. Once a patient population is chosen, the collaborative practice team is established with experts from nursing, medicine and physical therapy. Each team member will choose appropriate interventions and decide on specific outcomes within a specified time frame. (Sullivan, 2018. Page 37)
