Compare and Contrast
Right Sided Failure
Left Sided Failure
Blood goes back into the venous circulation
Most common. Blood goes back into the lungs / pulmonary circulation
Clinical manifestations:
Clinical manifestations:
-Jugular vein distention -Edema (first in lower extremities, then upper) -Hepatomegaly -Splenomegaly -Decrease in UO -Fatigue -Increased peripheral venous pressure -Ascites -GI distress/anorexia -Weight gain
-Manifests as pulmonary congestion and edema -SOB (& paroxysmal nocturnal dyspnea [elephant on chest], orthopnea) -Crackles -Congested cough -Pink frothy sputum (blood-tinged as the lungs fill up with blood more) -Tachypnea -Tachycardia – b/c problem with CO -Fatigue -Cyanosis -Elevated PAWP Results in acute decompensated heart failure (ADHF)
The primary cause of right-sided HF is left-sided HF. left-sided HF results in pulmonary HTN and congestion and eventually, chronic pulmonary HTN causes increased RV afterload which results in right-sided hypertrophy and HF. May result from an acute condition like a PE or right ventricular infarction; or cor pulmonale (right ventricular dilation and hypertrophy caused by pulmonary disease)...
