Gather all SUPPLIES -

Mask Clean gloves Sterile Gloves Dressing change kit (transparent dressing) Bio-patch Chlora-Prep x 1 Skin Prep Tape Pen

Don clean gloves and mask Provide pt with a mask Ask patient to turn head away from insertion site Inspect and palpate site around dressing for swelling Remove dressing by pulling it towards catheters insertion site 6. Assess site and examine catheter and hub “Looking for any drainage, redness, swelling, or foul odor” 7. Remove anti-microbial patch 8. Measure external portion of catheter to previous length to DETECT MIGRATION “Measuring external portion of catheter to previous length to detect migration” 9. Discard dressing and gloves 10. Perform hand hygiene 11. Open sterile dressing kit and don sterile gloves 12. Cleanse insertion site with ALCOHOL SWAB in circular motion for 30 seconds (ONE HAND [dirty hand] SHOULD BE HOLDING LINE AT ALL TIMES) 13. Cleanse skin with CHG swab under central line back and forth motion for 30 seconds 14. Cleanse central line with NEW CHG swab upwards for 30 seconds 15. Allow to air dry 16. Apply skin prep (Box formation around site) 17. Apply anti-microbial patch (bio-patch) 18. Apply transparent dressing

19. Coil external portion of catheter and tape in place 20. Label new dressing with date, time, and initials 21. Dispose of all soiled supplies in biohazard bag 22. Return bed to appropriate height, brakes locked, side rails up, call light within reach 23. Wash hands...
