Performance Assessment: Essentials of Advanced Nursing Roles and Interprofessional Practice ePortfolio – CAM2, Task One 1b. Summary of your advocacy action plan to address your identified SDOH Brief Summary: According to the data collected in 2017 and published by County Health Rankings & Roadmaps it showed that the prevalence of adult obesity (20 years and over with BMI >30) in Palm Beach County was at 22.2%, and the physical inactivity was 27%, which was higher when compared to Martin County, which was at 20%. and 23% respectively. (County Health Rankings & Roadmaps, n.d). According to Palm Beach County Community Health Improvement Plan (CHIP), obesity was the top second health priority identified in Florida. (Health Council of South Florida, 2020). National Center for Health Statistics (NCHS) in Figure 2 in their data brief, showed that the ageadjusted prevalence of obesity among adults aged 40 and over, by sex, race, and Hispanic origin was higher in non-Hispanic black women (56.9%) than non-Hispanic black men (44.1%). (Hales et al., 2020). Poverty in Palm Beach County was at 12.2%, which was higher than in Martin County (10.7%). (World Population Review, n.d.). Obesity led to many other chronic diseases that could have been prevented, such as sleep apnea, type II diabetes, coronary artery disease, cancer, hypertension, arthritis. (County Health Rankings & Roadmaps, n.d.). The prevalence of obesity poses an enormous clinical burden, extensive treatment, and care-delivery needs strategies and training to be adequately delivered for bariatric patients. There are multiple contributing factors such as poverty, lack of exercise, lack of education about healthy lifestyle, cheap and easy access to unhealthy food. (Florida Department

I will advocate for a policy change to have the access to reduced prices of healthy foods for the low socioeconomic status families and educate the community about the program and the locations of the stores participating in it. In this way, as a community, we can all work towards preventing adult obesity.

1c. Create a goal in SMART format to guide the Advocacy Action Team S (Specific)

Policy to reduce adult obesity in Palm Beach County by having access to Reduced Prices Healthy Corner Program, where local participating stores dedicate part of their healthy products to be sold at a lower price to help socioeconomically struggling families eat healthily. Florida Food Policy Council will be crucial in developing this program and involving the local stores because they provide a unique forum for diverse stakeholders to come together.

M (Measurable)

I would like to see a decrease in the percentage of obesity rate in the lowincome population and increase the number of people reporting healthier eating within the next six months. Also, measuring the number of stores participating helps to know if the targeted communities are being reached adequately.

A (Achievable)



I will ask the assistance of Florida Food Policy Council, to guide me on how to contact local stores.



I will learn how to prepare nutritious meal plans with the help of the local hospital dietician and develop a flyer. I will team with volunteers to distribute the flyers in schools, hospitals, shelters, worship places, stores and post them on social media. I will also include the location of participating stores.



I will look up the locations of the African Community and Conservation Foundation, and Black Chamber of Commerce of Palm Beach County to deliver flyers.

R (Realistic/Relevant) Reducing the obesity rate in Palm Beach County will prevent the increase of chronic diseases. T (Timebound)

1f. Brief reflection summary of my Phase 1 GoReact video reflection. From this section of the course, I learned that initiating a healthcare policy change involves many people like Congress, local officials, and personnel from the department of health. Each of them has different roles and capacities to make the change happen. I learned the importance of advocating to help reduce obesity in Palm Beach County because it was the top second health priority in Florida. I felt the motivation to change lives in my community and motivated volunteers who share my same passion. Every project has its challenges, but when I seek the help of professionals and more experienced resources, I was able to achieve my goal. I was able to organize my smart goal, which helped me focus on the target. In future advocacy planning, I will not waste my time trying to figure out things independently, but I will go directly to the organizations and personnel, who have more experience and influence, to guide me on the proper approach and channels to reach my goal.

Task 1, Phase Two Phase 2: Structuring the advocacy Action Team 2a. List seven (7) team-building strategies 1. Clear and respectful communication: Create a culture of open-door communication, respectful, transparent, and simple so everyone can understand it and provide a chance to ask questions. 2. Clear and achievable goals: Setting team goals is an effective method to achieve a desirable outcome efficiently. Team members should know what their responsibilities are and be held accountable and measure the progress. 3. Effective conflict management: A healthy culture is a crucial component to prevent conflicts at a workplace. Spotting early signs of conflict will help to resolve the situation before it escalates. Avoid taking sides but help the team members address the issues causing the conflict and reach a resolution that works for them. 4. Recognition and appreciation: Sincere appreciation and gratitude build trust. It will motivate others to step up their game to hear words of praise from their supervisors. Creating a recognition and rewards program in the workplace proved to be a strong incentive for employees to deliver their best performance. 5. Shared governance and effective decision-making: Encourage employees' engagement with decision-making because each one has a different level of competency and can influence and direct their coworkers to maximize team effectiveness.

6. Building trust among the team: Getting to know each other on a personal level helps to build trust. Sharing my failures with the team and being vulnerable encourages the team to be open. Never condescend but invite input from the others. 7. Create a cross-cultural and diverse team: Having team members from different generations will help the team be open-minded by learning about previous experiences and solutions that worked in the past. Also, the new generation with improved technology will help keep knowledge updated and communication more accessible. Respecting other cultures and learning about their values and beliefs teaches us to accept each other as we are. 2b. Consider the expertise you will need on your Advocacy Action Team to achieve your SMART goal: On my Advocacy Action Team, I will need: 1. The Mayor of Palm Beach County because she has the power to shape policies that foster sustainable, inclusive, and resilient food systems to help secure the future we want. 2. The board member from Florida Food Policy Council (FFPC) who will help to build partnerships between farmers and communities. 3. The Green Market Vendor Manager of Palm Beach County to get the registered locations of fresh market vendors. 4. The Dietician of the local hospital to help me develop balanced nutritious meal plans. 5. Health Council of Southeast Florida (HCSEF), the diverse group of stakeholders who meet to extensively review and discuss information to help assess, guide, and inform future planning efforts to meet the community needs.

Write a brief reflection summary of your Phase 2 GoReact video Brief Reflection Summary In this session, I learned how to address the team during the meeting and to set strategies for implementing our plans. I also learned about the importance of seven team-building strategies that help the team has clear and respectful communication. It is also vital to have clear and achievable goals to get a desirable outcome with the collaboration of the whole group. Conflict management is a crucial component to help resolve disagreements, misunderstandings and create smooth work fluidity. Shared governance will help the team have effective decision-making because different points of view can be summarized to create a solution with many members' input. Building trust among the team by getting to know each other personally will build a work/family relationship bond. Last but not least, it is important to understand the experienced generation and learn from their past solutions that worked out while the new generation can share the new knowledge to help form solid progress....
