Therapeutic Uses Increased cardiac output (positive inotropic effect) and slowing of the heart rate (negative chronotropic effect).

Adverse Effects

Nursing Interventions

CNS: fatigue, headache, weakness. EENT: blurred vision, yellow or green vision. CV: ARRHYTHMIAS, bradycardia, ECG changes, AV block, SA block.GI:anorexia, nausea, vomiting, diarrhea

Monitor apical pulse for 1 full min before administering. Monitor ECG throughout IV administration and 6 hr after each dose

Contraindications

Hypersensitivity; Uncontrolled ventricular arrhythmias; AV block; Idiopathic hypertrophic subaortic stenosis; Constrictive pericarditis; Known alcohol intolerance

Client Education

Teach patient to take pulse and to contact health care professional before taking medication if pulse rate is 60 or 100.

Medication/Food Interactions Thiazide and loop diuretics, piperacillin, ticarcillin, amphotericin B,corticosteroids, and excessive use of laxatives. Amiodarone, some benzodiazepines, cyclosporine, diphenoxylate, indomethacin, itraconazole, propafenone, quinidine, quinine,spironolactone, and verapamil

Evaluation of Medication Effectiveness

Medication Administration

IV (Adults): Digitalizing dose—0.5– 1 mg given as 50% of the dose initially and one quarter of the initial dose in each of 2 subsequent doses at 6-12 hr intervals

Decrease in severity of HF. Increase in cardiac output. Decrease in ventricular response in atrial tachyarrhythmias. Termination of paroxysmal atrial tachycardia.
