Underlying Principles Discharge planning Begins AT ADMISSION, and: - Should prepare for the functional ability of the client: this includes whether they have caregivers at home, or if they’re in need of one. - Should include an assessment of the client’s previous residence: if they’re returning there, identify if the dwelling need any structural adaptations or equipment. - A referral for social services can be made as needed to address gaps in the clients support system or resources. - Convey health status and needs to concerned parties and community service providers. - Includes documentation from the provider either approving or disapproving of cessation of care. Certain clients have the right to leave at any time

Nursing Interventions Transferring/discharging a client: 1. Confirm both the date and time of departure, as well as the validity of the clients future residence. 2.
