RESIN-BASED COMPOSITE Introduction to Enamel and Dentin Bonding

OBJECTIVES 1. State the compositional differences between enamel and dentin in terms of inorganic, organic and water content and why this is relevant to adhesive bonding. a. Enamel i. Mostly inorganic, some organic b. Dentin i. Mostly organic, some inorganic 2. Distinguish the adhesive classification system and be able to match adhesive application system description to its respective classification. a. Etch rinse – total etch i. 2 step ii. 3 step b. Self etching i. 2 steps ii. 1 step c. Universal i. 2 or 1 step 3. State the three basic steps in dentin adhesive resin placement and the function of each. a. Acid etch i. Remove smear layer and prepare the tooth for restoration, dissolves the inorganic content so that the collagen fibers are exposed and they can bind to and hold on to the restoration better b. Primer i. Puts coat of monomers on there and solvents c. Adhesive i. Makes hybrid layer with dentin and adhesive that increases the retention of the restoration. Hydrophobic monomers 4. State the basic composition of etchants, primers and adhesives (multiple or single bottles) and the role of each component in the adhesion to enamel and dentin. a. Etchants – acid b. Primers – monomers c. Adhesives 5. Identify and apply clinically what is meant by “moist” dentin after acid etching and why it is important a. . Moist – still have some water in there so that the surface is open and more 6. Recognize what is meant by “shiny” evenly primed dentin and why it is important.

a. Need to be evenly primed so that it is ready to accept the material and the composite 7. State what is the “hybrid layer” and why is it important. a. Hybrid layer i. Layer where there is some dentin and adhesive mixed in with it. makes the whole restoration and the dentin-composite bond stronger and more durable for the patient 8. Distinguish the critical steps in dentin adhesion and explain the proper technique of Optibond FL adhesive placement a. Etch i. Remove smear layer ii. Demin iii. Funnel shaped dentinal tubules iv. < 15 seconds b. Moist dentin c. Primer 30 s i. Air dry d. Thin layer of adhesive e. Light cure to get 18 J of energy 9. Describe and distinguish why bonding is important in dentistry and the clinical uses of this technique. a. 10. Recognize the challenges of enamel and dentin bonding. a. Enamel is hard because it is mostly inorganic and dentin is mostly organic so you need to be able to bond to 2 different materials in the same restoration b. The restorations need to be able to withstand immediate and long term stresses...
