Description of Skill Blood glucose monitoring uses a Glucometer that takes a small sample of blood to determine the concentration of free glucose in the blood. Turn on the machine put the test strip into the machine prick your finger and either wipe the first drop or blood off or not and collect one drop of blood onto the test strip and wait for the result.

Indications Type 1/ 2 diabetes Assess the puncture for bleeding/bruising, evaluate child and parent understanding of the procedure and test results, notify PCP of abnormal test results or administer insulin based on results

Outcomes/Evaluation Blood glucose level between 70-100 Compliance with glucose checks. Assess the puncture for bleeding/bruising, evaluate patients understanding of the procedure and test results, notify PCP of abnormal test results or administer insulin based on results.

Potential Complications infection, bleeding, bruising, sore/painful fingers.

Client Education use test strips that are recommended for the meter, take meter to PCP office so you can make sure you are measuring BS correctly, perform quality control checks, clean the meter, discuss importance of maintaining glycemic control, teach the patient the procedure, explain how to dispose of lancets

Nursing Interventions Assess site before and after, assess child meds (are they taking anticoagulants), use alcohol wipes, always wear gloves, make sure machine is on, test strips aren't expired or inappropriate for the machine and that adequate sample size is obtained...
