assessment 05/07/2020 E portfolio What have you learned so far in this course that will help you conduct effective health assessments? So far, I have learned about the complete health history of an adult and that it consists of a patient: biographic data, source history, present health, present illnesses, past health, last examination date, allergies, current medication family history and a review of systems. I also learned how to ask the appropriate questions during an interview, such as avoiding “why” questions and the 10 traps. I learned that the purpose of a health history is to establish a baseline of client’s health status and it is important to let the client know about the purpose during an interview. I learned that about the note taking drawbacks, for example note taking limits eye contact with the client and the nurse can miss valuable nonverbal cues. I also learned about the pain scale that will health me identify and document the clients pain level. I learned about different cultures and how I should asses my own beliefs before interviewing a client. What skills do you have you gained? I gained the skill of giving an interview in order to obtain the health history of a client; through the interview I learned how to listen more and talk less. I learned about inspection, palpation, percussion and auscultation. I learned that inspection requires good lighting, adequate

exposure and sometimes instruments such as an otoscope, penlight and ophthalmoscope is used. When palpating I learned that it is best to use your fingertips for fine tactile discrimination of skin texture. I learned when assessing the abdomen, you should palate last. Describe any areas that are still unclear and ways you will gain clarity. I tend to get confused when trying to differentiate from percussion and auscultation, I will gain clarity by doing more research and practicing the skill. When listening to abdominal sounds I always forget how many seconds I should listen to each quadrant; I will gain clarity by looking in the book and practicing. I have trouble remembering the cranial nerves, I will gain clarity by watching YouTube videos and practicing....
