Mental Health Case: Li Na Chen, Part 1 Guided Reflection Questions Opening Questions How did the simulated experience of Li Na Chen’s case make you feel? I thought it was difficult to choose the correct responses. If it were in real life, I would have been able to come up with my own repsonses and established a therapeutic relationship with the patient. Talk about what went well in the scenario. I thought I did a good job at filling out the mental examination form. I also thought I picked the right questions to ask Li Na during this simulation. Reflecting on Li Na Chen’s case, were there any actions you would do differently? If so, what were these actions and why? I would have chosen better responses to what Li Na was saying. There were occasions where I chose the wrong response that caused her to shut down even more. Scenario Analysis Questions* PCC What priority problem(s) did you identify for Li Na Chen? The priority problems I identified were risk of suicide and hopelessness. Her husband also showed signs of hopelessness. Mrs. Chen had a decreased affect, had a hard time thinking straight, had a loss of interest, and was very tired. What potential problems could arise due to the major depression? Potential problems that could arise due to major depression is suicide or poor self care. Her children could also experience neglect because she seems to not be able to get out of bed to take care of them. Her husband is also facing problems due to her depression and it seems to be affecting his mental health as well. S/EBP Identify potential patient safety interventions related to Li Na Chen’s suicide attempt. Any objects that could cause self-harm need to be removed from her room. It is good that her husband is in the room with her because she needs to be monitored and the nurse should also make frequent visits to her room. This patient’s mood and thoughts should also be regulated. PCC/ T&C

Discuss Li Na Chen’s developmental stage according to Erikson and the importance of individual and group counseling with other adults who have attempted suicide.

Mrs. Chen is struggling to find her purpose in life. She has no self-worth as she repeatedly said she “is nothing.” Mrs. Chen is going to need counseling to remind her of what she has to live for and find healthy coping mechanisms. Group counseling will also help because she will realize she is not the only one going through this and she will feel that someone understands her situation. EBP

How could the change in the medication regimen have impacted Li Na Chen’s mental state? Trying another medication can always have a bad outcome. It seems that she could have been switched to a new medication that was not working as well as her sertraline was which could have been why she began feeling strong symptoms of her depression again. Also, her nurse practioner was out of town and was unable to assess how her new medication was working for her.

In Li Na Chen’s situation, what therapeutic communication techniques would be most effective? When communicating with Mrs. Chen, she seemed to respond best to the communication technique of restating. When I restated and validated how she was feeling, she opened up more. In other instances when I used other techniques, she seemed to shut down.

Concluding Questions Describe how you would apply the knowledge and skills that you obtained in Li Na Chen’s case to an actual patient care situation. I learned a lot about the effectiveness of restating. Restating helps the patient feel that you are listening. Active listening is a huge component to the nurse-patient relationship and that is something I would use clinically in the future.
