PURPOSE OF MEDICATION

Expected Pharmacological Action Inhibits vasoconstrictive and aldosterone-secreting action of angiotensin II by blocking angiotensin II receptor on the surface of vascular smooth muscle and other tissue cells.

Therapeutic Use Lowering of BP in hypertensive patients.Decreased progression of diabetic nephropathy.Decreased incidence of stroke in patients with hypertension and left ventricular hypertrophy

Complications CNS: dizziness, asthenia, fatigue, headache, insomnia. CV: edema, chest pain. EENT: nasal congestion, sinusitis, pharyngitis, sinus disorder. GI: abdominal pain, nausea, diarrhea, dyspepsia. Musculoskeletal: muscle cramps, myalgia, back or leg pain. Respiratory: cough, URI. Other: angioedema.

Medication Administration Tablets: 25 mg, 50 mg, 100 mg

Contraindications/Precautions Contraindicated in patients hypersensitive to drug. Use cautiously in patients with impaired renal or hepatic function. Dialyzable drug: No. Overdose S&S: Hypotension, tachycardia, bradycardia.

Interactions Drug-herb. Ma huang: May decrease antihypertensive effects. Discourage use together. Drug-food. Salt substitutes containing potassium: May cause hyperkalemia. Monitor patient closely.

Nursing Interventions Drug can be used alone or with other antihypertensives. Monitor patients who are also taking diuretics for symptomatic hypotension. Regularly assess patient's renal function (via creatinine and BUN levels). Patients with severe HF whose renal function depends on the angiotensin-aldosterone system may develop acute renal failure during therapy. Closely monitor patient's BP, renal function, and potassium levels, especially during first few weeks of therapy and after dosage adjustments.
