Description

Part III Mary Keeper, a 41- year old patient, complains of repeated headaches and problems sleeping within one month. She then reports to her physician, Dr. Nee. The patient is a carrier for hepatitis B, and suffers from arthritis. Keeper also takes the medications Celebrex and Glucosamine. When visiting Dr. Nee, he ordered a blood chemistry, thyroid, and endocrine test in order to understand the problem that Keeper is facing. A week later, Marry returned to the office to hear her results. The doctor informed her that she may be suffering from an endocrine dysfunction, Dr. Nee had some concerns about her T3 and T4 values. Making note that Keeper has lost around 10 pounds over 10 months. After getting the results back Dr. Nee noticed that her cholesterol was very high 269 mg/dL, and that her calcium levels have increased. Dr. Nee could conclude that Keeper may be suffering from hyperthyroidism. As stated in the Mayo Clinic report that hyperthyroidism occurs when your thyroid gland produces too much thyroxine, this can accelerate your body's metabolism (Mayo Clinic). Looking deeper, Dr. Nee could also conclude that grave disease could also be what is wrong with Keeper. Graves disease is connected to hyperthyroidism, the symptoms include: weight loss despite normal eating habits, and enlarged thyroid. Another diagnostic Dr. Nee could conclude that it is Pituitary adenoma, which in this case could be benign. Pituitary adenoma arises from the pituitary gland, accounting for 15% of primary brain tumors. Symptoms include headaches, vision loss, and bleeding into the tumor. Within Keeper’s blood chemistry results, it showed that her calcium levels, parathyroid hormone values, and cholesterol levels were exceedingly high compared to the rest of her blood chemistry results. Dr. Nee could also focus on the other results such as osmolarity. The University of Rochester Medical Center explains osmolarity as the concentration of dissolved particles in your blood, the test help diagnose a fluid or electrolyte imbalance, including dehydration. Added to the list is the LH hormone, being that it is very high wish could result in her estrogen levels being high. As stated in part 2, her T3 and T4 levels were high, since being high, there are plenty of thyroid issues that Mary could have such as grave disease,

hyperthyroidism, and nodular goiter. Nodular goiter is when lumos develop in the thyroid gland, which contain abnormal thyroid tissue causing excess hormone to be made. Dr. Nee could diagnose her hyperthyroidism as primary hyperthyroidism. Mary's symptoms can lead to a pituitary tumor. The rationale behind Mary’s headaches could be because of a tumor eroding in the sinus area or brain cavities. This can cause pressure in the brain or the tumor’s effect on the hormones can cause headaches. Mary is experiencing headaches because the tumor could be putting pressure on the optic nerve resulting in poor vision.
