Workshop Activities: Activity 1: Modified Barthel Index (MBI) 1) Read through the information provided in the assessment pack and review your lecture notes to assist in answering the following questions: 

What is the name of the assessment?



What client group is it designed for?



What client groups is it not appropriate for?



What ADLs does it assess?



Does the assessment have evidence of reliability



Does the assessment have evidence of validity



Consider the utility of the assessment. Are the instructions easy to follow? How long does it take to complete? Is scoring easy to complete? Is training required to conduct the assessment?



Describe the type of normative data available for interpreting the results?



How can the results of this test assist in assessing the client’s occupational performance

2) Watch through another video clip of an OT conducting an interview with a client to complete the MBI.



Take note of the questions and communication style the OT is using to gather the information

 Score the MBI based on the responses the client is providing Activity 2: Applying the Modified Barthel Index (MBI) (via Interview)

1. Working in pairs, each student takes a turn to role play the client, either Mrs Land or Mr Waters.  Practice your interviewing, communication and rapport building skills including: o Introduce yourself, the OT role, the purpose of assessment and how it will be conducted o Be aware of your body language and positioning o Utilise active listening, reframing and open ended or probing questioning o Allow for time for the ‘client’ student to answer  The ‘Client’ student may interpret and expand on the information provided in the case study to fit the questioning  ‘OT’ student fills out/scores the MBI form as the interview progresses  Total score is calculated at the end of the interview Swap roles using the second case study.

2. At the end of each interview, reflect with each other on your interviewing technique – what worked? What didn’t? What could be improved?

MBI Case Study 1: Mrs Land

Mrs Land has experienced a left CVA (cerebral vascular accident) that has resulted in weakness to her right hand & arm (dominant) and lower limb. She also has impairment of sensation (touch, pain, temperature & proprioception) on the right side of her body. Mrs Land reports that she is most concerned about the loss of use of her right upper limb. She is finding daily tasks such as dressing, washing & styling her hair difficult. She is concerned that she will have to be helped with self care tasks. She would like to be able to return to her previous level of functioning (she was previously independent) – the thought of being dependent is concerning her. Use the information below about Mrs Land to help inform the MBI interview: 

She has difficulty reaching and holding objects (e.g. toothbrush) with her right hand



She has difficulty manipulating objects with her right hand – tends to use left hand most of the time. Occasionally uses right hand when prompted but goes back to one handed use fairly quickly



Difficulty co-ordinating in bilateral motor tasks e.g. using a knife and fork. Her food has to be cut up for her.



Difficulty locating objects on her right side as she does not always “see” objects. Tends to disregard objects falling, spilling on right side



She is continent in both bowel and bladder



She is able to walk around the hospital using a forearm rollator walker



She needs assistance with transfers



She is currently showering with the use of a shower chair and HHSH



Her lack of use and neglect of her right arm is due to muscle weakness (hemiplegia) and sensory impairment.

MBI Case Study 2: Mr Waters

Mr Waters is 88 years old and was admitted to hospital following a fall at home. His wife has reported that her husband’s function as being declining over the past few months. Mr Waters is concerned about his ability to complete his self care activities. He is finding tasks such as dressing and showering difficult. He does not want to be a burden on his wife. Use the information below about Mr Waters to help inform the MBI interview: 

Able to walk around his house using a wheeled walker. Does not go out much as he has difficulties walking longer distances



No difficulties with eating - able to cut up food



Has difficulties getting on and off the toilet due to the low toilet height. Otherwise he is able to toilet himself.



Able to use his dominant hand to complete tasks such as brushing teeth, combing hair etc. with no difficulties



Nurses have being supervising him transferring in and out of the shower. Able to shower himself once seated on a shower chair



Able to put on shirts independently. However, has some instability when putting on pants.



Continent in bowel and bladder...
