Short Term Goal: After a 2-3 hours of intervention:  patient will be able to discuss how current life stressors have affected normal coping strategies.  patient will be able to articulate ability to cope and ask for help when needed.  patient will be able to communicate needs with others to be able to meet his needs.  Patient will be able to remain free of vicious behavior towards others or self.

Independent Nursing Interventions:

Long Term Goal: After 1-4 days of intervention the patient will be able to:  Achieve satisfactory performance with coping mechanisms and self-love.  Patient will be able to show ability participate in some or all levels socially.

1. Medication administration as ordered.

1. work with patient to set realistic goals and identify personal knowledge and skills of coping. 2. Stimulate patient to participate and make choice in planning ahead. 3. Supervise patient for risk of harming others or self and interfere appropriately. Dependent Interventions (Physician Order Required)

2. work on getting proper order for counseling if it’s needed. 3. Allow time to relax and be supportive of coping behaviors.

Scientific Rationale 1. Involving the patient in making decisions helps with their independence. 2. By the patient participating in the development of the care plan, it gives him increased self-esteem and the feeling of control. 3. Observed for causes of ineffective coping, such as lack of support in recent changes with situations in his life.

1. Allows him to relief the symptoms experienced. 2. Assist in arranging referrals for the patient to work with the interdisciplinary team and resources to help him develop the necessary coping skills and problem-solving techniques. 3. A supportive presence creates a safe and supportive environment. To enhance any coping technique.

Evaluation/ Reassessment Evaluation of Short-term goal: (With re-assessment statement) -

The patient was able to acknowledge and discuss the stressors in his life and was able to communicate and participate well with the team of providers. Patient continued to not have any destructive behavior.

Evaluation of Long-term goal: (With re-assessment statement) -

Patient was able to demonstrate a suitable amount of participation during the intervention.
