GENERIC NAME: Orlistat

TRADE NAMES: Alli, Xenical

MOA: this medication works by irreversibly inhibiting the gastric and pancreatic lipases. They play an important role whe digesting dietary fat. INDICATIONS: include patients with obesity who are at risk for hypertension, diabetes and dyslipidemias CONTRAINDICATIONS/PRECAUTIONS: potential chronic malabsorption, cholestasis, anorexia INTERACTIONS with other Drugs/Substances: interferes with warfarin, amiodarone, cyclosporine, and thyroxine. These medications affects their bioavailability and effectiveness PATIENT RESPONSE / REACTIONS: DESIRED OUTCOME: This medication is used to help patients maintain a healthy body weight. It does so by blocking fat from being absorbed into the body.

COMMON SIDE EFFECTS: loose stools, urgent bowel movements, diarrhea

ADVERSE REACTIONS: inability to control bowel movements, liver damage,

S/S TOXICITY: orlistat may inhibit carboxylesterase which is a major detoxification enzyme which may cause severe liver, pancreatic and kidney damage

TOXICITY/OVERDO E TX: liver, pancreatic and kidney damage

NURSING ACTIONS: Helps with weight loss, decreases the absorption of dietary fat

ASSESS: Prior to administering the medication the nurse must assess the patient. Gathering all medical history and lab reports is very important. Be sure the patient isn’t on any medications that may interfere with orlistat

CARE/IMPLEMENT: Should take a 120 mg pill three times a day with meals containing fat

TEACH: Educate the patient on how often the pill should be taken. This medication is meant to help people lose weight so making sure the patient is eating properly is very important.

EVALUATE: Seeing how the patient reacts to this medication is very important. If the patient has a allergic reaction it’ important to note
