What is peritonitis o Inflammation of the peritoneum (lining of the abdominal cavity) o Usually due to bacterial infection, but may occur secondary to fungal or mycobacteria o Most common is E. coli, klebsiella, proteus, pseudomonas, strep o Can come from abdominal surgery/trauma o Inflammation that extends to an organ outside of the peritoneal area, such as kidney or peritoneal dialysis S/S o At first is diffuse pain, then becomes constant, localized, and more intense over the site o Movement aggravates o Tender, distended and ridged abdomen o N, V, decrease peristalsis, fever, increase pulse, hypotensive, shock, sepsis Assessment/Dx o WBC, bands o H/H o Electrolytes o Abdomen x-ray shows air and fluid o Ultrasound o Paracentesis o CT Medical Management o Increase isotonic fluid due to hypovolemia o Pain meds, antiemetics, suction, oxygen, IV antibiotics, identify infection and treat Nursing Intervention o Signs that peritonitis is subsiding: decrease in temp and pulse, abdomen is softening, return of bowel sounds, passing gas and bowel movements o High risk for bleeding and infection...
