(a) You are the regular pharmacist at a small independent pharmacy. On the way to work there is an accident so you will be ten minutes late. You phone the pharmacy and speak to your dispenser. She says a regular patient is outside wanting to buy a 32 pack of paracetamol. Can she open the pharmacy and sell the paracetamol? Please give your rationale for your response. (2 marks)

(b) A patient on a business trip from Hong Kong comes to your pharmacy and says he has run out of his ant hypertensive medication. He asks if he can get a small amount as an emergency supply. Can you make this supply? Please give your rationale for your response. (2 marks) (c) You receive a veterinary prescription for a human medicine. You have the medicine in stock. What words are needed on the prescription for this supply to be made? If no words were written what legal category of medicine would need to be supplied? (2 marks) (Total: 6 marks) 4.

You are working in a community pharmacy which provides a methadone dispensing service for people with substance abuse problems. One of your regular clients comes in on a Saturday to collect their instalment prescription for their weekend supply. The prescription is for dispensing 3 x 20 ml doses of Methadone DTF 1mg/ml on Friday. The prescriber has failed to add any endorsement with regard to missed collection to the prescription. He tells you that yesterday on Friday morning his wife went into labour and he was with her all day at the hospital and was unable to collect his prescription. He is now the proud dad of a new son. He asks you to please dispense him his Saturday and Sunday doses as if he doesn't get his Methadone for the weekend he will be ill by Monday and unable to go into work and he is likely to lose his job. You know that he is working, that his wife was pregnant and her due date was around now. In general he is a committed and motivated client you know to be truthful. When using a structured approach to solve an ethical dilemma, one of the steps is to identify the stakeholders and the likely impact upon them. In considering whether to supply the Methadone to him or not, who are the stakeholders and the likely impact upon them? (6 marks)

Answer ALL THREE parts to this question: You are about to carry out an MUR with a patient. You have printed off a copy of their patient medication record (PMR). (a) Outline all the physical steps you would take to protect the patient’s information during the MUR process, and keep this confidential afterwards. (8 marks) (b) Which legal act regulates the process of processing personal data? (1 mark) Your pharmacy is a very busy one, carrying out many services. You have genuine concerns that a fellow pharmacist is taking patient data and using this in an unlawful way. (c) Outline why it is important to raise concerns, and the steps you would take to raise this concern, including who would you talk to and where you could go for support (11 marks) (Total: 20 marks)

Answer ALL THREE parts of this question: (a) The first Francis Report into the situation at Mid Staffs Foundation NHS Trust identified a range of problems or failings that were systematic throughout the hospitals. Describe what these were. (8 marks) (b) In his report Lord Francis said it should be patients, not numbers, that should be counted and the patient should be put first day in and day out. He made a number of recommendations on how NHS organisations should achieve this. What are they? (8 marks) (c) The failings at Mid Staffs Foundation NHS Trust were primarily due to failings in nursing care. However, as a clinical service, pharmacy and pharmacists also have a duty in ensuring the patient is put first. With particular reference to the recommendations outline two ways in which hospital pharmacy services and/or hospital pharmacists can ensure that patients are put first.
