Description

CLASS: CNS Stimulant GENERIC NAME: Phentermine TRADE NAMES: Adipex-P, Lomaira MOA: Decreases hunger by altering the chemical control of nerve impulse transmission in the appetite control center of the hypothalamus. INDICATIONS: Short-term treatment of obesity in conjunction with other interventions (dietary restriction, exercise); used to produce and maintain weight loss in patients with a BMI ≥30 kg/m2 or ≥27 kg/m2 in the presence of other risk factors (diabetes, hypertension, hyperlipidemia). CONTRAINDICATIONS/PRECAUTIONS: Hypersensitivity or known intolerance to sympathomimetic amines or tartrazine; Cardiovascular disease; Hyperthyroidism; Uncontrolled hypertension; History of drug abuse; Agitation; Glaucoma; Concurrent or recent (within 14 days) MAO inhibitor therapy; Concurrent SSRI antidepressants. OB: Lactation: Pregnancy or lactation. Use Cautiously in: Diabetes mellitus; Concurrent use of other weight-loss drugs. Pedi: Children ≤16 yr (safety and effectiveness not established). INTERACTIONS with other Drugs/Substances: Drug-Drug: Concurrent use with MAO inhibitors may result in hypertensive crisis and is contraindicated (do not use within 14 days of MAO inhibitors). Increase risk of adverse CNS events with alcohol. Concurrent use with SSRI antidepressants is not recommended. May decrease insulin or oral hypoglycemic requirements in patients with diabetes. Topiramate may increase levels. PATIENT RESPONSE / REACTIONS: COMMON SIDE EFFECTS: ADVERSE REACTIONS: S/S TOXICITY: TOXICITY/OVERDOSE TX: CV: VALVULAR DESIRED OUTCOME: CNS: CNS stimulation, ABNORMALITIES Appetite suppression with resultant confusion, dizziness, RESP: PULMONARY weight loss. dysphoria, euphoria, HYPERTENSION headache, insomnia, mental depression, restlessness. EENT: blurred vision. CV: hypertension, palpitations, tachycardia. GI: constipation, diarrhea, dry mouth, nausea, unpleasant taste, vomiting. GU: changes in libido, erectile dysfunction. NURSING ACTIONS:

ASSESS: CARE/IMPLEMENT: Monitor patients for weight PO: Administer 30 min before meals or as a single loss and adjust concurrent dose before breakfast or medications 10-14 hr before retiring.
