Evaluate the third factor of domino theory which established by Herbert W Heinrich and give an example for each. (2 marks) Unsafe act – do maintenance without stopping the machine Unsafe condition – no proper SOP or sign for machine’s maintenance / no LOTO

Assume you are the Safety Officer of that company, by referring to OSHA 1994, give FOUR (4) relevant sections which related to the person responsible or the action to be taken. Justify your answer. (8 marks) Section 15- general duties of employers –to provide a safe and health workplace, provide a correct procedure, give a proper training how to do proper cleaning Section 16- to develop safety and health policy- employers need to have an OSH policy to show their commitment and seriousness in OSH isssues.
