Medical Case 5: Skyler Hansen Guided Reflection Questions 1. How did the scenario make you feel? a. This scenario made me realize you won’t always have orders when you get a patient. Starting by getting vitals is always a smart option to give the doctor a better ISBARR. 2. What management options would have been appropriate if Skyler Hansen had been alert and could swallow? a. I would’ve given the patient the carbohydrates and protein to eat first. This includes glucose tablets, fruit juices, or hard candies. 3. If Skyler Hansen’s acute hypoglycemic episode had not have been treated immediately, what could have happened? a. Skyler could’ve had a continued to lose consciousness, become drowsy, lethargic, and possibly have a seizure. He could’ve also fell into a diabetic coma. 4. If too much glucose were administered to Skyler Hansen while the health care team was trying to correct his blood glucose level, what could occur? a. Skyler could’ve went into hyperglycemia which has symptoms like fatigue, headaches, and blurred vision. 5. What key elements would you include in the handoff report for this patient? Consider the SBAR (situation, background, assessment, recommendation) format. a. Skyler is a 18 year old male brought in for low blood glucose while playing basketball. He states he hadn’t eaten 5 hours prior to the incident. Skyler is diagnosed with Type 1 diabetes. Vitals were as follows: HR 92, R 19, O2 97%, T 99F, BP 126/76. Initial blood glucose level of 66 mg/dL and was given 50mL D5W in water IV and carbohydrates orally per physicians orders. His glucose level rose to 202 mg/dL. 6. Describe age-appropriate patient teaching for Skyler Hansen and resources that may be helpful to him. a. Skyler will need some reading material that includes what Type 1 diabetes is, how to manage it, and recommendations on what and when to eat. Giving background on the disease will help him to better understand his situation and share it with those who can help. 7. Discuss confidentiality and legal empowerment of 18-year-old patients such as in Skyler Hansen’s case. a. Because Skylar is 18, he can have patient and physician confidentiality of his own. That meaning he can choose who o share this information with. It will not be directly shared with his parents without his permission. 8. What would you do differently if you were to repeat this scenario? How would your patient care change? a. I would work faster on beginning the vitals. That way I could call the provider and get the orders faster. This would limit the amount of time of Skyler’s low blood glucose.
