DX: Post/op Gastric Bypass
1. Discuss the importance of frequent vital signs post op, including comparison with previous results. The importance is because all patients remain at risk for pneumonia, shock, cardiac arrest, respiratory arrest, clotting, VTE (venous thromboembolism) and GI bleeding. (Ignatavicius, pg 258). Taking frequent vital signs and comparing them with previous results helps the nurse know if something is wrong with the patient and catch the problem before it becomes worse.
2. Identify six signs of blood loss. A. Mild tachycardia B. Hypotension C. Narrowing of pulse pressure D. Respiratory rate increases E. Vasoconstriction in the skin making it pale and cool to the touch F. Anxious, confused and lethargic. (Murray, pg 602-603) 3. List and prioritize three nursing interventions for a patient with suspected hypovolemia. A. Oxygen therapy B. IV therapy C. Drug therapy (Ignatavicius, pg 747-748) 4. List pertinent lab values to be monitored post operatively. SpO2- >95%, ABG, electrolytes, CBC (complete blood count), WBCs, creatinine
