INSTRUCTIONS: 1. For each chapter that teaches about medications, fill in the following table regarding the chosen meds. CLASS: Anticholinergics GENERIC NAME: Tolterodine TRADE NAMES: Detrol, Detrol LA MOA: relaxes smooth muscles in urinary tract by inhibiting acetylcholine at postganglionic sites INDICATIONS: used for overactive bladder (frequency, urgency) and urinary incontinence, decreases symptoms of overactive bladder CONTRAINDICATIONS/PRECAUTIONS: contraindications include hypersensitivity, uncontrolled closed-angle glaucoma, urinary retention, gastric retention; precautions include pregnancy, breastfeeding, children, renal/hepatic disease, controlled closed-angle glaucoma, bladder obstruction, QT prolongation, decreased GI motility INTERACTIONS with other Drugs/Substances: drug-drug interactions with chloroquine, clarithromycin, droperidol, erythromycin, grepafloxacin, halofantrine, haloperidol, methadone, and pentamidine may increase QT prolongation; do not use fesoterodine in those with known hypersensitivity PATIENT RESPONSE / REACTIONS: COMMON SIDE EFFECTS: ADVERSE REACTIONS: S/S TOXICITY: TOXICITY/OVERDOSE TX: paresthesia, fatigue, headache, anxiety, dizziness, nausea, DESIRED OUTCOME: dry mouth, blurred vision, none increasing dementia, memory vomiting, anorexia decreased urinary frequency, fast heartbeat impairment, chest pain, urgency hypertension, vision abnormalities, xerophthalmia, abdominal pain, constipation, dry mouth, dyspepsia, dysuria, retention, frequency, UTI, rash pruritus, bronchitis, cough, pharyngitis, upper respiratory tract infection

NURSING ACTIONS: When taking tolterodine, beer should be avoided in older adults with/at high risk for delirium frequency. Tolterodine should only be used by pregnant women only if benefits outweigh fetal risk and should not breastfeed.

ASSESS: •





urinary patters such as distension, nocturia, frequency, urgency, incontinence, residual urine angioedema, StevensJohnson syndrome, allergic reactions, rash (if any of these occur than discontinue product) ECG for QT prolongation, ejection fraction, chest pain, palpitations, dyspnea, (may be compounded in those taking class IA/III dysrythmics)

CARE/IMPLEMENT: swallow whole, give with liquids

TEACH: • advise patient to avoid hazardous activities, dizziness may occur, shortness of breath, urinary retention • advise patient not to drink liquids before bedtime, to swallow extended-release product whole • Teach patient importance of bladder maintenance. Teach patient to take with liquids and swallow whole. Notify prescriber is pregnancy

EVALUATE: therapeutic outcome is decreased urinary frequency, urgency
