Medication Management Worksheet Medication Name (Brand & Generic) Generic: Aripiprazole Brand: Abilify Classification Antipsychotic, mood stabilizer Is prescribed dose safe? Adults: IM 400mg PO; after first injection, continue treatment with oral aripiprazole for 14 days; if no adverse reactions to 400mg/month dose, may lower dose to 300 mg every month. Pt’s diagnosis: Schizophrenia, bipolar disorder, major depression, and Tourette syndrome Drug Action or Purpose Psychotropic activity may be due to agnostic activity at dopamine and serotonin receptors. Desired Effect (What are your patient’s target symptoms?)  Decreased manifestations of schizophrenia.  Decreased mania in bipolar patients.  Decreased symptoms of depression.  Decreased agitation associated with schizophrenia or bipolar disorder.  Decreased emotional and behavioral symptoms of irritability. Unwanted Effects (Risks and side effects)  Increased mortality in elderly patients with dementia-related psychosis and suicidal thoughts and behaviors with antidepressant drugs.  Suicidal thoughts, drowsiness, extrapyramidal reactions, confusion, depression, fatigue, hostility, insomnia, constipation, agranulocytosis, tremors, hypersensitivity reactions.  Tardive dyskinesia, high blood sugar in those with diabetes, neuroleptic malignant syndrome. Key Patient Teaching Points or Instructions  Excreted in breast milk; discontinue drug or begin bottle-feeding.  Use cautiously if patient has known cardiovascular or cerebrovascular disease. Conditions with cause hypotension (dehydration, treatment with anti-hypertensives, or diuretics); diabetes may increase risk of hyperglycemia; seizure disorders; patients at risk for aspiration pneumonia.  Unclear whether it is safe or effective in people less than 18 years old.  Drink plenty of water while taking this medication.  Do not drink alcohol.

Adults: Initial dose 10 or 15mg PO once a day. Maximum dose 20mg orally once a day. Pt’s diagnosis: Schizophrenia, bipolar disorder, sometimes used in combination with other medications to treat depression. Drug Action or Purpose The exact action of olanzapine is unknown, but it is thought to effect activity of dopamine and serotonin receptors. Desired Effect (What are your patient’s target symptoms?)  Decreased manifestations of schizophrenia (hallucinations)  Decreased feelings of agitation Unwanted Effects (Risks and side effects)  Increased mortality in elderly patients with dementia-related psychosis and suicidal thoughts and behaviors with antidepressant drugs.  Back pain, constipation, dry mouth, headache, increased appetite, light-headedness, nausea, sore throat, stuffy nose, tiredness, vomiting, weight gain, missed menstrual periods, decreased sexual ability, and orthostatic hypotension.  Rarely causes tardive dyskinesia or neuroleptic malignant syndrome. Excreted in breast milk; discontinue drug or begin bottle-feeding. May cause withdrawal symptoms in a newborn if taken within the last 3 months of pregnancy. Some medicines may interact with olanzapine: tramadol, alpha-blockers, hypertension medication, anticholinergic, carbamazepine, HIV protease inhibitors, omeprazole, dopamine receptor agonists. If patient has diabetes they should use this medication with caution because it may raise blood sugar levels. Do not drink alcohol. Olanzapine may lower the ability of the body to fight infection. Avoid contact with people who have colds or infections. Do not become overheated or dehydrated in hot weather or while being active; heatstroke may occur. This medication causes you to sweat less.
