Expected Pharmacological Action -Suppresses inflammation and normal immune response. -Has numerous metabolic effects. -Suppresses adrenal function at chronic doses at 4mg/day. -Has negligible mineralocorticoid activity.

Therapeutic Use -Suppresses inflammation and modification of the normal immune response. -Replacement therapy in adrenal insufficiency.

Complications -Peptic ulcerations -Thrombolism -Hypertension -Fluid retention -Hypokalemic -Alkalosis

Medication Administration This medication is available in PO, IM, IV

Contraindications/Precautions Contraindicated: -Active untreated infections -Epidural use -Known alcohol, bisulfite, cow's milk, tartrazine hypersensitivity or intolerance -Administration of liver virus vaccines -Lactation Precaution: -Chronic treatment -Stress -Systemic sclerosis -Potential infections may mask signs

Interactions -Loop diuretics -Hyperkalemia may increase risk of digoxin toxicity. -May increase requirement of insulin or oral hypoglycemic agents. -Phenytoin, phenobarbitual, rifampin stimulate metabolism and may decrease effectiveness. -Increase risk of GI bleed.

Evaluation of Medication Effectiveness Decreased in presenting symptoms with systemic side effects.

Nursing Interventions -Assess involved systems before and periodically during therapy. -Assess patients for signs of adrenal insufficiency before and periodically during therapy. -Monitor intake and output ratios and daily weight gain. -Monitor for rales and crackles and dyspnea.

Client Education -Instruct patient on correct technique of medication administration. -Glucocorticoids cause immunosuppression and may mask symptoms of infection....
