PURPOSE OF MEDICATION
Expected Pharmacological Action Interferes with hepatic synthesis of vitamin K-dependent clotting factors 2,7,9 & 10
Therapeutic Use Prevention of thromboebolic events
Complications dermal necrosis, cramps, nausea, calciphylaxis, bleeding, fever
Medication Administration Dosage - PO 2-5mg daily for 2-4 then adjust per INR
Contraindications/Precautions uncontrolled bleeding, open wounds, active ulcer, brain, eye or spinal cord injury or surgery, severe hepatic or renal disease, uncontrolled hypertension, fetal hemorrhage
Interactions abciximab, androgens, estrogens, capecitabine, cefotetan, chloramphenicol,disulfiram, fluconazole, fluoroquinolones, itraconazole, metronidazole, thrombolytics, NSAIDs, valporates, aspirin, alcohol, barbituates, carbamazepine, St Johns wort, anise, arnica, chamomile, clove, dong quai, fenugreek, feverfew, garlic, ginger, ginko, panax ginseng, licorice, foods high in vitamin K
Evaluation of Medication Effectiveness - Lab value monitoring - Increased INR of 2-4.5 - Increased PT 1.3-2.0 times control

Nursing Interventions - Assess vitals before and during therapy - Monitor for signs of bleeding - Assess for signs of thrombosis - Monitor INR & PT ranges - Evaluate hepatic function before and during treatment - Withhold if INR or PT esceed ranges
Client Education - Take medications as prescribed - Educate on administration - Report signs and symptoms of bleeding ie. tar like stoll, bruising w/o injury, unusual swelling, nose bleeds, etc. - Educate on signs of thromboembolism and to go to hospital. - Avoid IM injections - Avoid food high in vitamin K - Avoid activities that could cause injury
