Bronchodilator, Adrenergic Beta 2 agonist, sympathomimetic CATEGORY CLASS ______________________________________________________________________ PURPOSE OF MEDICATION

Expected Pharmacological Action Bronchodilation by action on beta 2 receptors by increasing levels pf cAMP, which relaxes smooth muscle; produces bronchodilation, CNS or cardiac stimulation as well as increase diuresis and gastric acid secretions; longer acting than isoproterenol.

Therapeutic Use Prevention of exercise-induced asthma, acute bronchospasm, bronchitis, emphysema

Complications Tremors, anxiety, restlessness, angina, dry nose, paradoxical bronchospasm.

Medication Administration Inhalation and PO

Contraindications/Precautions Hypersensitivity to sympathomimetics. Precaution: Pregnancy, cardiac renal disease, hyperthyroidism, diabete mellitus, angle-closure glaucoma, seizures, tachydysrhythmias

Interactions Increase: digoxin level - digoxin CNS stimulation - CNS stimulates ECG changes - potassium-losing diuretics Server hypotension - oxytocics Toxicity - theophylline Action of bronchodilators action of albuterol - tricyclics, MAOIs, other adrenergics; avoid use together. Decrease albuterol - other Beta blockers

Evaluation of Medication Effectiveness absence of dyspnea, wheezing improved airway exchange, improved ABGs/VBGs.

Nursing Interventions Monitor V/S: Spo2, HR, respirations with lung sounds and BP. ABGs CBC,CMP

Client Education Use as prescribed. if you missed a dose. take it once you remember you missed it and alter schedule to new dosing schedule. avoid OTC unless ok'd by PCP. Use of inhaler and go over insert with patient. Demonstrate the correct usage of inhaler. do not wash, wipe with dry cloth. limit caffeine intake.
