GENERIC/BRAND NAMES Alendronate Fosamax Fosamax plus

DRUG CLASS/KEY INDICATIONS Bisphosphonates - Prevention and treatment of osteoporosis (including postmenopausal and corticosteroidinduced)

KEY DRUG INTERACTIONS

Paget’s disease: 40mg once daily for up to 6 months

- Take in the morning with a full glass of plain water and at least 30 mins before any food or drink or any other medicines

- Consider dental assessment before starting bisphosphonates and always let the dentist know if taking this medication before any dental work is done as can cause osteonecrosis

- Bisphosphonates + antacids/calcium/iro n/magnesium (They reduce alendronates absorption therefore wait AT LEAST 30 mins but 2 hours is advised)

Treatment of postmenopausal osteoporosis: 10mg once daily; or 70mgonce weekly

- Treatment of osteoporosis when D3 supplementation is also recommended

Prevention of postmenopausal osteoporosis: 5mg once daily

- Paget’s disease of bone

Treatment and prevention of corticosteroid-induced osteoporosis: 5mg once daily; or 10mg once daily for postmenopausal women not receiving oestrogen In combination with colecalciferol (D3): 1 tablet once weekly

- Must remain upright for 30 mins to avoid oesophageal burn or gastric upset - Do not take within 30mins of antacids, iron, calcium or mineral supplements as it may interfere with alendronates absorption - Do not chew suck tablet. Must be swallowed whole - Stop taking and see DR immediately if you have pain on swallowing or new or worsening heartburn.

- Avoid in pregnancy and breast feeding – no data - Dose adjustment required in renal impairment. Avoid in severe renal impairment as reduced renal function has been reported - vomiting, nausea, diarrhoea, headaches, hypocalcaemia, musculoskeletal pain - Monitor if taking NSAIDS as can increase risk of ulcers

- Alendronate and NSAIDS (increases risk of gastric ulcers)
