Opening Questions How did the simulated experience of Amelia Sung’s case make you feel? This Vsim was very exciting and the closest I have gotten to a birth so far. I wish we were able to get a clinical rotation in labor and delivery because it was a very beautiful and exciting scernaio. Describe the actions you felt went well in this scenario. I wasn’t expecting the baby to be delivered in the scenario but I think I adapted well to the situation. I wasn’t done with my assessments or following the providers orders when the doctor came in saying she was going to deliver the baby but I think I did well adapting to the situation. No orders were provided about how to prepare for birth, and this is not something we went over in class but overall I adapted really well.

Scenario Analysis Questions1 EBP What risk factors specific to shoulder dystocia were identified in Amelia Sung’s case? Risk factors include Ms. Sung’s excessive weight gain, her prolonged second stage, and the positive turtle sign indicating shoulder dystocia. PCC/I Prioritize your nursing actions for Amelia Sung based on your assessment. My first priority would be to preform a vaginal examination to see where the fetus is. Once this was done, crowning was seen so I knew to call the provider right away and prepare for delivery. It would be important to monitor the fetal heart rate and provide O2. It would also be important to follow the providers instructions during delivery by putting her in the knee to chest position and applying subrapubic pressure. EBP

List potential problems for Amelia Sung and her baby related to the shoulder dystocia.

Concluding Questions Reflecting on Amelia Sung’s case, were there any actions you would do differently? Explain. I would have been less focused on getting all of the providers orders completed and would have called the provider right when I saw the babies head was crowning. Describe how you would apply the knowledge and skills that you obtained in Amelia Sung’s case to an actual patient care situation. I will now have more understanding of what to do during a birth. I know what turle sign means which is not something I felt I really understood and I will know what position to place the mother in if there is a positive turle sign, and how to apply subrapubic pressure.
