Opening Questions How did the simulated experience of Amelia Sung’s case make you feel? I felt good during the case. I would also say stressed, this was the first OB vsim we have had and there were new things to perform and assess, and during these assessments the baby started being delivered and I wasn’t sure whether to focus on the delivery or the assessments still. Also, we have not gotten any of the maternity information yet so I will admit I was a little bit lost. I was also excited though, because this is the material I have been most excited to learn. Describe the actions you felt went well in this scenario. The actions I feel went well in the scenario was just following the list of orders initially, it was mainly assessments that needed to be done, for example the vital signs and the head to toe. Also, I believe the delivery went well, the pre quiz had really helped me in understanding what needed to be done in this case, specifically the step stool and applying suprapubic pressure.

Scenario Analysis Questions1 EBP What risk factors specific to shoulder dystocia were identified in Amelia Sung’s case? The risk factors were the mothers excessive weight gain, the prolonged labor, and the positive turtle sign that happen while the baby was trying to be delivered. PCC/I Prioritize your nursing actions for Amelia Sung based on your assessment. My top priority would have been a vaginal examination, I had not gotten to it yet before the provider showed up for delivery. Also maintain adequate oxygenation for the mother and monitoring the fetal heart rate are also top priorities. The next top priority I would say is following the providers instructions and putting the mother into McRoberts position, and applying suprapubic pressure. EBP

Potential problems for the baby could’ve included a broken clavicle, or asphyxiation, a broken arm or possible nerve damage.

List the interprofessional health care team that should be involved in Amelia Sung’s case.

The nurse, the anesthesiologist, the NICU and the obstetrician would all be important in the involvement of Amelia’s case. S/QI/I Consider what points might be included in a safety checklist for shoulder dystocia. The mothers condition, the fetal heart rate, the position of the fetus, and the procedures that may need to be done should all be included in the safety checklist.

Concluding Questions Reflecting on Amelia Sung’s case, were there any actions you would do differently? Explain. Actions I would do differently is perform the vaginal exam before worrying about the head to toe, I would probably do it right after vitals. I also would have been able to call the provider sooner. Another thing I would do as soon as we were aware of the shoulder dystocia was call the NICU. Describe how you would apply the knowledge and skills that you obtained in Amelia Sung’s case to an actual patient care situation. I will now have a little bit more knowledge and understanding of what to do during a birth. Specifically, if a baby ever has a positive turtle sign, I will know what procedures or actions are going to need to be taken. (McRobert’s position and applying suprapubic pressure).
