Guided reflection questions: Opening Questions How did the simulated experience of Sandra Littlefield’s case make you feel? I felt a little judgemental of the patient at first. She has many large tattoos and dyed red hair which makes her appear angry. At the beginning she was displeasant which made me not want to work with her. Talk about what went well in the scenario. In this scenario the refocusing went well. I found it was a good response to refocus the patient on herself and her treatment when she demonstrated poor personal boundaries. Reflecting on Sandra Littlefield’s case, were there any actions you would do differently? If so, what were these actions and why? I would have liked to spend more time talking about coping with pain with this patient. Its impossible for me to know if she is actually feeling pain or med seeking so I have to trust and believe what the patient tells me about their pain, but I would have liked to offer more interventions for coping with pain other than just medications. Scenario Analysis Questions* PCC/S What should be the nurse’s priority when caring for Sandra Littlefield? When caring for patients the priority is always safety. With this patient a safety concern is the potential for violence as she has a history of being violent. The nurse should follow all safety protocols to ensure that both the nurse and the patient are safe. What action should be initiated to maintain personal safety? To maintain personal safety the nurse should remain a safe distance away from the patient and she should not allow the patient to stand between herself and the door or the exit. If the nurse did not recognize the importance of personal safety, what could be the consequence? The consequence could be an escalation of violence without intervention leading to severe injury or death of the nurse. PCC/S Discuss the importance of therapeutic communication and setting limits for Sandra Littlefield. Therapeutic communication and setting limits are especially important for patients diagnosed with borderline personality disorder. These patients often have poor personal boundaries and try to elicit inappropriate personal information from staff members. In these instances it’s important to redirect the patient and the conversation towards focusing on the patient and their treatment. T&C/S What members of the healthcare team should be involved in Sandra Littlefield’s case? The doctor should be involved in managing her pain, prescriptions, and diagnostic tests regarding her back pain. The nurse is involved in connecting the patient to other resources such as the library, providing patient education, and coordinating with the doctor. Concluding Questions Describe how you would apply the knowledge and skills that you obtained in Sandra Littlefield’s case to an actual patient care situation. In an actual patient care situation it’s extremely important that I don’t divulge personal information to the patient. This is important not only from a professional standpoint, but also important for therapeutic communication. Sharing personal information reduces the efficacy of
