Opening Questions How did the simulated experience of Fatime Sanogo’s case make you feel? I felt rushed during the simulation just a little bit because I knew from the prequiz that the scenario was going to involve a postpartum hemorrhage. It makes it simpler to follow the orders the doctor gives and it is also helpful to go through the patient’s chart before beginning the scenario. Describe the actions you felt went well in this scenario. I believe my assessments went well, they were done in an organized manner as well as in a timely manner. Also the medication administration and contacting the provider at appropriate times went well too. Scenario Analysis Questions1 EBP/I What risk factors did you identify in Fatime Sanogo’s case that could have led to postpartum hemorrhage? Risk factors included her being past her due date (41 and 4/7 weeks), her delivery being an induction with oxytocin. Also she had a prolonged second stage of delivery, as well as a large baby that weighed 9 pounds that was born within the last 24 hours, and she had a second degree laceration. EBP

List the actions that should be initiated if administering oxytocin is ineffective.

The actions that need to be taken would be to decrease or control bleeding and prevent the patient from going into hemorrhagic shock. Lactated ringers bolus, as well as communication with the provider to be able administer other medications to help the uterus to contract. EBP

What were the assessment findings in Fatime Sanogo’s case that indicated her condition was worsening?

Her heart rate kept increasing and her blood pressure kept decreasing, indicating that she was going into hemorrhagic shock. PCC

What communication techniques could be initiated to decrease the anxiety of Fatime Sanogo and her husband and provide care that is culturally sensitive?

The nurse could have provided a translator although in the scenario it did not seem to be an issue. The nurse would also try to make sure to be culturally competent in the care she is providing to the patient and her family. T&C

How important do you feel it is to include Fatime Sanogo’s husband as a member of the health care team? Why?

It would be extremely important to include her husband because the baby is also his, and education would not only go to the mother, it would include him as well. Also, if the mother did deteriorate further, the husband would be the advocate for his wife.

Concluding Questions Reflecting on Fatime Sanogo’s case, were there any actions you would do differently? Explain. In real life, I would have gotten a translator, in the v-sim it did not seem to be an issue. Describe how you would apply the knowledge and skills that you obtained in Fatime Sanogo’s case to an actual patient care situation. This v-sim would help me in knowing how to care for a patient that is at risk for a post-partum hemorrhage and patients that develop post-partum hemorrhages after vaginal deliveries.
