Description of Skill APGAR Scoring permits a rapid assessment of the need for resuscitation based on five signs, indicating the physiologic state of a neonate.

Indications The birth of a child - every neonate recieves an apgar score. Performed at 1 minute and 5 minutes after birth. If score is persistently low, 10 minute assessment can also be performed. The APGAR acts as a baseline for all future assessments.

Outcomes/Evaluation A - Appearance P - Pulse G - "Grimace" or response to stimulation A - Activity R - Respiration Each area is scored as 0, 1, or 2. A score between 7-10 indicates that the infant is having no difficulty adjusting to life outside of the uterus.

Potential Complications Incorrectly assigned apgar scores can lead to necessary interventions being missed, leading to further complications for the infant.

Nursing Interventions (pre, intra, post) The nurse is responsible for the care of the newborn immediately after birth. Verify that respiration has been established. Dry the infant, assess temperature, and place identification bracelet. ASsess heart rate, based on auscultation.

Client Education Tell the parent that the baby will be assessed and given a score. Inform the parent of the score and what this can potentially mean for the baby.

Nursing Interventions Always be prepared with emergency equipment to provide infant resuscitation, in the case of complications. Also assess reflexes, jaundice, and reaction to cold stress within the first 2 days of life. Maintain proficiency in performing APGAR scoring...
