Ramipril is an ACE inhibitor that lowers blood pressure by blocking the conversion of angiotensin I to angiotensin II and is indicated for hypertension. It is not indicated for Dennis as renal impairment increases the risk of hyperkalaemia [1]. Metformin is indicated for use in type 2 diabetes to assist with the lowering of blood glucose levels. Patients requiring metformin are typically overweight, including Dennis who has a BMI of 35.1indicating obesity [2]. It is apparent that with a creatinine clearance (CrCl) of 10 mL per minute, Dennis should not be using metformin [3]. Simvastatin inhibits the enzyme that synthesises cholesterol thus reducing lipids. Due to Dennis’ renal impairment this drug is contraindicated [4]. Aspirin inhibits cyclooxygenase by limiting the synthesis of prostaglandins. When used in low doses it decreases the risk of a heart attack or stroke.
