Expected Pharmacological Action prevents conversion of fibrinogen and prothrombin to thrombin by enhancing inhibitory effects of antithrombin III (ATI, p. 191)
Therapeutic Use prevention of DVT, PE, MI, open heart surgery, A-FIB, anticoagulant in dialysis and transfusion procedures, treatment of DIC. (ATI, p. 191)
Complications Hemorrhage, thrombocytopenia, anemia, herperin induced TCP (ATI, p. 191)
Medication Administration IV infusion IV bolus cannot be used interchangeably (unit for unit) with LMWHs or heparinoids. SUBCUT abdomen between pelvic bones, rotate sites.
Contraindications/Precautions
avoid all IM injections that may cause hematoma (ATI, p. 192)
contraindications: bleeding, hypersensitivity precautions: children/elderly, diabetes, renal disease, severe HTN, pregnancy, lactation (ATI, p. 192)
Interactions NSAIDS, aspirin, and other anticoagulants because they can increase the risk of bleeding (ATI, p. 192)
Nursing Interventions platelet count q2-3 days. bleeding, hemorrhage; gums, petechiae, black tarry stools, decrease in hematocrit, check periodically for sign of decreasing clots. PTT, occult blood in stools. (ATI, p. 192)
Client Education Evaluation of Medication Effectiveness prevention of DVT and pulmonary emboli adequate coagulation based on aPTT (ATI, p. 193)
