ATI MENTAL HEALTH B 2019 PTSD  Assessment o I would assess for a history of a traumatic event (emotionally, naturally, in combat, or vicariously through a family member or close friend). I would assess the patients present coping mechanisms and past treatments. Because patients with PTSD are at a higher risk for other mental disorders, I would also assess for signs and symptoms of dissociative disorders, anxiety, depression and substance use disorders  Incidence o Post-traumatic stress disorder is a common mental health disorder due to the large possibilities of events that could cause a patient’s psyche to be extremely disturbed.  Nursing implications o It is very important to establish a therapeutic relationship with the patient to build trust between the healthcare provider and the patient during times of stress. I would provide a safe and non-threatening environment and use different tools to decrease any anxiety. I would also screen the patient for suicidal ideation and take the necessary precautions if needed (assign a one to one) OPIOID WITHDRAWAL  Assessment o I would assess for withdrawal manifestations. These include sweating, rhinorrhea, goosebumps, tremors and irritability. Later severe weakness comes with diarrhea, fever, insomnia, pupil dilation, N/V, muscle and bone pain and muscle spasms. I would assess for last date used, patterns of frequency of use and any previous periods of abstinence.  Incidence o A substance disorder is diagnosed if a patient is using a substance for at least 12 months and it causes clinically significant impairment. The number of patients that willingly go into withdrawal is significantly low because many patients use denial  Nursing implications o Understand that withdrawal is a very unpleasant process for the patient to go through but that it is not life threatening. Educate the patient on medications that can help them go through their withdrawal like Methadone, clonidine, buprenorphine, and naltrexone SCHIZOAFFECTIVE DISORDER  Assessment





o Assess for signs and symptoms of schizophrenia, both positive and negative symptoms, and for signs and symptoms of a mood disorder like depression, mania or mixed episodes. Incidence o The incidence of schizoaffective is very low but it is more common in women than men. Nursing implications o Establish a strong trusting therapeutic relationship and create a safe environment standing at least an arm’s length away from the patient. Assess the patient for suicidal ideation and provide the appropriate precautions if needed....
