Hailey Lawson Professor Kenzie Lee NURS4000 14 January 2019 Community Case: Health Literacy Case Part 1 Ernesto, a 53-year-old Hispanic male, was admitted to the hospital with severe hyperglycemia and is now newly diagnosed with type 2 diabetes. Ernesto’s wife died 2 years ago, and he lives alone, next door to his adult son, Marcos. Ernesto speaks little English and was discharged from the hospital after 3 days. The discharge nurse at the hospital instructed Ernesto on his new medications without Marcos present because the bed was needed for another client. As the nurse instructed him, Ernesto nodded yes to the instructions. Questions for students: 1. What is your overall opinion of the discharge process by the hospital nurse? State three areas of priority concerns. a. I would not feel comfortable with a nurse who did not question if the patient spoke a language that required a medical interpreter. It seems that the nurse was not aware of his health literacy level, which puts the patient at risk. Thirdly, there was no investigation to his disease, or if he understood how to modify his lifestyle to better his health. 2. Are you comfortable that Ernesto understood the discharge instructions? What could have been done differently? a. I am not comfortable with the description of this discharge process. The nurse should have taken time to advocate for her patient and his understanding of this disease. There should have been a medical translator present to explain the disease process, medications, and lifestyle changes. The fact that he nodded along does not mean he had a firm understanding of the instructions given to him. To prevent this misinterpretation, he should have taught back what he learned to the nurse before being discharged. 3. Which ethnicities are more prone to a diabetes diagnosis? a. African Americans, Mexican Americans, and Native Americans are a few ethnicities that are more prone to diabetes. 4. What are three specific areas of follow-up care that you anticipate for Ernesto? a. In regard to communication, assess understanding by asking what questions the patient has, and ask the patient repeat instructions to the interpreter. b. Concerning Health Literacy, ensure that written information is clear and easy to understand. Materials written at a sixth- to eighth-grade level help most patients.
