NURSING DIAGNOSIS

Impaired Dentition related to deficient knowledge regarding dental health as manifested by presence of tooth caries

PLANNING

At the end of 4 hours of rendering nursing care, the client will be able to: 1.Verbalize and demonstrate effective dental hygiene skills 2. Report adequate nutritional and fluid intake

IMPLEMENTATION

RATIONALE

To promote wellness: Instruct client or significant other in home-care interventions Recommend to limit sugary and highcarbohydrate foods in diet and snacks

Advise mother to teach the child in brushing their teeth 3. Follow through on while young referrals for appropriate dental care

To treat condition and prevent further complications To reduce buildup of plaque and risk of cavities caused by acids associated with breakdown of sugar and starch
