A 70 year old man with HTN and DM admitted to medical ward with the attempt of suicide. Patient’s family stated that the old man used to say that he is not useful any more. He is less expressive and talks less. He lost appetite and never sits with the old age people around him. Moreover, he feels desperate and this is reason for his attempt to suicide. It was observed that the patient was unable to sleep properly his blood glucose levels are raised. Patient used to cry and show low mood and unable to concentrate. Moreover, patient is unable to remember the recent things. Upon exploration patient explained that he don’t want to live as he is not worthy and his presence and absence does not make any difference. Moreover, his family and children don’t need him and he is a burden upon them. These are the statements he usually hear from his sons and his daughter in laws. The nursing assessment showed that the patient is with depression and he shared a suicidal plan with the nurse as well. During conversation patient showed less eye contact and withdrawn behavior. Patient showed a behavior of rejection and no interest in the social activities. Assessment Subjective data: I am not worthy, The social activities don’t provide me pleasure at all, Family don’t like me due to my illness and current condition, People don’t want to have a contact with me Objective data: Less eye contact Crying when asked about the friends Depression No proper speech

Nursing Diagnosis Social isolation related to fear of rejection secondary to the social phobia (depression)

Nursing Care Plan for the patient: Goals Nursing Interventions Short term Goal: The patient will be able to express the sorrow and loss

Long Term Goal: The patient will be able to get involved in the process of socialization and different activities (playing, laughter Social therapy ) and with isolation related to loss family members of usual social and society members after contacts discharge from hospital

a) Active listening helped a trust full relationship with the client b) Involvement of the family in the care process also helped in promoting the socialization c) Old friends and colleagues were asked to help the old man again in social activities. d) Group therapy helped the client to understand the importance of the socialization e) Involved in cognitive activities to help in remembering and focusing f) Effective communication helped in exploration of feelings and providing care accordingly. g) Teachings and practices were taught to the patient to cope with such situations in the future h) Patient and family agreed to do the same practices at home
